The Health and Housing Case Management Program is a partnership between Alliance, Duke University, and RHD North Carolina.
An Alliance study of health care costs at Duke University Hospital found that the vast majority of costs were driven by a fairly small number of people. After identifying those “heavy users,” RHD provided housing and case management — and quickly reduced hospitalizations by more than 40 percent.

Bridge housing supports a Housing First approach by providing safe and temporary housing for people who are actively seeking permanent supportive housing options and may face additional challenges. This 3-5 month “bridge to independence” program is intended for a more vulnerable population who are exiting jails and other institutions or living in long-term homelessness.
While in the program, a person is connected to additional services and community resources, receives assistance with housing search and navigation, and coaching to help increase their self-sufficiency whether it be employment, educational attainment, daily living skills, or budgeting.
The goal of the program is to help a person begin their journey of recovery and community living while putting all necessary wraparound services in place for them to be successful and live the lives they desire.
